Non-Medical Collaborative Care
Human-Centric Support for Serious Illness and End of Life
Who We Help
Your circle of care is the community that surrounds you in a time of change, loss, and uncertainty. Our doulas help guide and support that community so it can better support you.
At Present for You, we work directly with those who are ill or dying and their families. We also work with the wider care team—doctors, nurses, hospice providers—so that everyone is working together in service of our clients’ comfort and wishes.
We collaborate closely with hospice programs to ensure people receive the best possible care at the end of life, with both their medical and non-medical needs met.
Individuals & Families
Every journey is unique. Our support adapts to you and your family’s specific non-medical needs.
We can:
- Sit at the bedside for conversation or quiet companionship.
- Assist with legacy projects to capture memories and meaning.
- Provide respite for caregivers who need time to rest and recharge.
- Offer emotional and spiritual support for both you and those you love.
Providers
We believe the best outcomes happen when medical and non-medical care work together seamlessly.Through open communication and collaboration, we partner with physicians, nurses, and other healthcare providers to:
- Support not just the patient but their entire family, however they define support.
- Smooth the transition between medical and non-medical care.
- Ensure that emotional, spiritual, and practical needs are addressed alongside medical treatment.
Hospice
Present for You works hand-in-hand with hospice teams to extend the depth of care for patients and families. Our end-of-life doulas provide:
- Emotional, spiritual, and practical support tailored to each individual.
- Presence and guidance for families during the final days and hours.
- Support for hospice teams so they can focus entirely on their clinical care.
Payers
Unmet emotional, spiritual, and practical needs during serious illness often lead to unnecessary—and costly—medical interventions.By addressing these concerns early, we can:
- Prevent avoidable hospitalizations and emergency room visits.
- Improve the quality of life for patients and families.
- Increase satisfaction for members and subscribers.
